The Medicaid Service Coordination program at New Visions is a Medicaid State Plan service. The service coordinator helps the person and his or her family to develop, implement and maintain the person's life plan, to advocate for him or her in all areas of life, especially in the two main areas of protecting and upholding personal rights and ensuring that the living environment is safe.
